The Role
As a Support Coordinator, you’ll play a vital role in helping NDIS participants understand and implement their individualised plans. You'll provide tailored support and guidance while also leading a small team to ensure high-quality service delivery.
Your responsibilities will include:
- Allocating referrals efficiently to appropriate services
- Facilitating the intake process and coordinating service agreements
- Building strong relationships with clients, families, and key stakeholders
- Ensuring services are aligned with NDIS goals and funding availability
- Providing face-to-face support and case management for participants
- Offering advice and assistance to navigate the NDIS
- Recording all client communications and service updates in the internal system
- Supporting and mentoring team members to foster professional growth
Requirements
- At least 2 years' experience in NDIS Support Coordination or Community Case Management
- Deep understanding of NDIS principles and person-centred practice
- Demonstrated ability to work independently and within a team
- Strong written, verbal, and interpersonal communication skills
- Confidence in liaising with service providers, networks, and families
- Ability to manage client budgets, track outcomes, and maintain records
- Passion for supporting people with disabilities to reach their goals
You will need:
- 100 points of identification
- A valid Australian Driver’s Licence and a fully insured vehicle
- NDIS Worker Screening Check
- Working With Children’s Check
- Full working rights in Australia
