The Role:

Coordinate and provide culturally appropriate care through Visiting and Specialist Services. Key responsibilities include:

  • Collaborating with the multi-disciplinary team to deliver high-quality prevention and treatment
  • Identifying and prioritising clients’ needs in partnership with Health Programs and the Community
  • Developing strong referral pathways and ensuring comprehensive care
  • Coordinating Specialist Services, Case Conferences, Telehealth, and Outreach Visits

Requirements:

  • Registered Nurse with AHPRA Registration
  • Immunisation, ALS, REC or MEC, Pharmacotherapeutic Certs
  • Manual Driver’s License
  • Satisfactory Criminal History Check
  • NT Working with Children Clearance OCHRE card
  • Min 5 years nursing experience post-registration, with 3 years in a remote primary health field
