The Role:
The Support Coordinator plays a crucial role in coordinating NDIS participants and supporting them to achieve the goals outlined in their plans. This is a dynamic and people-focused position ideal for someone who is self-motivated, results-driven, and passionate about making a difference.
Key responsibilities include:
- Working with participants to achieve their goals in line with their NDIS plan
- Coordinating and maintaining services to ensure quality, continuity, and participant satisfaction
- Engaging professionally with all stakeholders, including families, providers, and clinical teams
- Maintaining accurate documentation and administrative records
- Supporting the implementation of policies, procedures, and operational improvements
- Staying up to date with NDIS systems, compliance, and best practices
Requirements:
- Minimum 2 years of experience in Support Coordination or related NDIS roles
- Strong understanding of person-centred care and NDIS processes
- Excellent communication and stakeholder engagement skills
- Ability to manage competing priorities in a fast-paced environment
- Relevant qualifications in Disability, Community Services or similar (desirable)
