Within this role, you will connect NDIS participants to supports and services within the Community. You will support a caseload of consumers with disabilities to increase their capacity to understand, monitor and manage the supports of their choice. You will increase consumer engagement in the planning and delivery of their supports whilst providing an exceptional experience and promoting choice.
Criteria:
- Experience across Disability, NDIS/Mental Health
- Proven experience developing creative solutions to overcome day to day challenges for customers
- Proven experience working in a fast-paced environment where you need to be adaptable, responsive and timely with stakeholder engagement
- Strong interpersonal and communication skills to build and maintain effective relationships.
- Experience providing and/or knowledge of Coordination of Supports is highly desirable
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans
- NSW Working with Children Check (WWCC)
- NDIS Worker Screening Check
- Worker Orientation Module ‘Quality, Safety and You
- COVID 19 Vaccination in line with Health Orders
- Willingness to use personal vehicle for work purposes
