The role:
You will coordinate safe, responsive and person-centred supports for an allocated caseload of NDIS participants, building strong relationships with participants and families to understand their goals, needs and preferences. You will assist participants to understand and utilise their NDIS plans, exercise choice and control, and connect with appropriate funded, mainstream, community and informal supports.
You will coordinate and monitor services across providers and stakeholders, ensuring supports remain aligned with participant goals and changing needs. This includes reviewing Care Plans, Risk Assessments and Service Agreements, supporting NDIS plan reviews, preparing reports and documentation, monitoring budgets and advocating for appropriate supports. You will work closely with intake, rostering, finance and clinical teams to ensure smooth onboarding and continuity of care.
The role also involves maintaining strong relationships with support workers, allied health professionals, families and service providers, while providing guidance and mentoring to allocated support staff. You will maintain accurate case notes and documentation, ensure compliance with NDIS and safeguarding requirements, manage risks and appropriately escalate incidents or high-risk concerns, while promoting participant independence, inclusion and quality of life.
Requirements:
- Qualification in disability, allied health, social science, welfare, community services (or equivalent relevant experience)
- Experience in support coordination within disability or community services
- Working knowledge of the NDIS, documentation, incidents and safeguarding
- Strong communication, organisation and systems capability (CRM/MS Office)
- Current driver licence; able to do home visits as required
- Current checks as required (NDIS Worker Screening, WWCC, police check)
