The Role:
The client is a value- based, young, dynamic, for-profit NDIS provider. As a Support Coordinator you will provide varying levels of coordination to NDIS participants. Your primary focus is to enable an individual to build their capacity (and that of their networks) with the aim of independently managing their issues and supports whilst meeting their goals and exercising choice in obtaining supports that assist them in pursuing their own lifestyles.

Responsibilities and duties:
  • Maintain a Support Coordination case load as agreed with the supervisor and achieve claimable hours as discussed and agreed and provide case management service.
  • Be involved in special projects as needed.
  • Assist NDIS participants in their plan implementation.
  • Work within funding level associated to each participant.
  • Facilitate effective working relationships and communications with the carers, family, external services and other key stake holders.
  • Communicate effectively with a wide variety of people both in the organisation and the community.
  • Linking clients to service providers to achieve their NDIS goals.
  • Continually gather and document relevant information about participants from other services for the development of evidence-based reports and updates.
  • Assist participants to prepare for plan review meetings and to build their capacity for independent living. Empower clients to exercise choice and control to access a range of formal and informal mainstream and community-based support, by identifying opportunities for innovative service delivery.
  • Continually seek opportunities to build the capacity of clients and their families enabling greater levels of independence.
  • Conduct risk assessment; develop support plans and shift plans for participants.
  • Comply with the organisations policies and procedures, and with funding guidelines.
  • Follow Occupational Health and Safety standards, guidelines, and practices.  
Your Skills
  • A relevant tertiary qualification
  • An understanding of NDIS
  • Experience as a NDIS Support Coordinator (Desirable)
  • Working with Vulnerable People Care and a current Police Clearance Certificate (done within last three months)
  • Proven ability to work individually and as part of a team to motivate, support and communicate effectively with people who are at risk of harm and/or exhibiting challenging behaviours.
  • A current driver license and a car with comprehensive care insurance
