As a Social Worker, you will play a key role in delivering high-quality support and coordination services to individuals and families. Your responsibilities will include:

Providing psychosocial disability (mental health) supports.
-
Completing plan review reports, biopsychosocial, and family assessments.
-
Developing individual and budget plans, and implementing them to achieve personal goals.

Collaborating with allied health professionals and coordinating service delivery.
-
Offering support for individuals, families, and those experiencing social disadvantage.
-
Providing Coordination of Supports or Specialised Support Coordination to ensure participants receive the right level of care.
The successful candidate will bring:

A Bachelor’s degree in Social Work.

Current AASW registration.
-
Strong oral, written, and interpersonal communication skills.
-
The ability to work independently and as part of a team.
-
Active listening skills, empathy, and a client-focused approach.

Well-developed organisational skills, including time management and prioritisation.

Current Working with Children Check, NDIS Worker Screening, Police Check (within 6 months), Driver’s Licence, car insurance, and a roadworthy vehicle.

Computer literacy, with proficiency in Microsoft Office and Excel.
Desirable:

Experience working with clients with dual diagnoses, or in forensic disability/forensic mental health.

Knowledge of the NDIS and experience working with NDIS participants.

Willingness and ability to travel by car.

Experience working with culturally and linguistically diverse (CALD) people and their support networks.
