The Role

Our client is seeking a dedicated and experienced  Service Coordinator  to manage the daily operations of their in-home and growing clinical care services.

This role is pivotal in ensuring the delivery of high-quality, efficient, and compliant care services. The successful candidate will be execution-focused, thrive in fast-paced operational settings, and demonstrate a passion for healthcare excellence, team development, and continuous improvement.

Key responsibilities include:

  • Managing day-to-day operations and ensuring services are tailored to participants’ needs, goals, and choices
  • Building strong relationships with participants and families, reviewing care plans, and responding to feedback
  • Overseeing scheduling and rostering to ensure continuity of care and service efficiency
  • Supporting implementation and continuous improvement of policies, procedures, and strategies
  • Ensuring compliance with accreditation standards, laws, and internal benchmarks
  • Identifying and resolving operational challenges to minimise disruption to services


Requirements

  • Experience in disability services, care coordination, or case management (NDIS experience highly regarded)
  • Relevant qualifications in Disability, Community Services, or similar (desirable)
  • Strong knowledge of person-centred care and NDIS processes
  • Excellent leadership, communication, and organisational skills
  • Understanding of healthcare regulations and accreditation standards
  • Ability to manage multiple priorities in a fast-paced environment
