The Role
Our client is seeking a dedicated and experienced Service Coordinator to manage the daily operations of their in-home and growing clinical care services.
This role is pivotal in ensuring the delivery of high-quality, efficient, and compliant care services. The successful candidate will be execution-focused, thrive in fast-paced operational settings, and demonstrate a passion for healthcare excellence, team development, and continuous improvement.
Key responsibilities include:
- Managing day-to-day operations and ensuring services are tailored to participants’ needs, goals, and choices
- Building strong relationships with participants and families, reviewing care plans, and responding to feedback
- Overseeing scheduling and rostering to ensure continuity of care and service efficiency
- Supporting implementation and continuous improvement of policies, procedures, and strategies
- Ensuring compliance with accreditation standards, laws, and internal benchmarks
- Identifying and resolving operational challenges to minimise disruption to services
Requirements
- Experience in disability services, care coordination, or case management (NDIS experience highly regarded)
- Relevant qualifications in Disability, Community Services, or similar (desirable)
- Strong knowledge of person-centred care and NDIS processes
- Excellent leadership, communication, and organisational skills
- Understanding of healthcare regulations and accreditation standards
- Ability to manage multiple priorities in a fast-paced environment
