The role:
As a Senior Care Partner, you will play a crucial role in overseeing and managing the allocation and scheduling of support workers to participants receiving care at home. Your responsibilities include developing efficient rostering systems that balance staff availability, skills, and participant preferences to provide timely and uninterrupted care. You will work collaboratively with care management teams and families to ensure care plans are tailored and responsive to individual needs.

In addition to rostering, you will serve as the main contact for participants and their families, addressing concerns and providing guidance to support workers. Maintaining compliance with relevant regulations, tracking service delivery, and driving ongoing improvements in rostering processes will also be key to your success in this role.

Requirements:
  • Tertiary qualification in Health or related field
  • Experience in care management, rostering, or support-at-home services
  • Strong understanding of Aged Care and NDIS compliance standards
  • Excellent communication and interpersonal skills
  • Current National Police Check, Working with Children Check, and NDIS Worker Screening Check (or willingness to obtain)
  • Current First Aid and CPR certifications
  • Minimum two doses of COVID-19 vaccination
  • Right to work unrestricted in Australia
