The role
Provide high-quality, person-centred care to residents across both aged care and retirement living services. Deliver support primarily focused on the Transition Care Program to support residents after a hospital stay. Assisting with daily living, wellness and social engagement. Collaborate with a multidisciplinary team to maintain a safe welcoming environment. This role is primarily community based. There will be some need for home visits to assess suitability for residents returning home.
Requirements

Occupational Therapy Degree + AHPRA registration

Strong communication and interpersonal skills
