The role:
In this role, you will deliver evidence-based occupational therapy services to adult and Aged Care clients, supporting them to maintain independence, safety, and quality of life across home and community settings. You will complete functional assessments, equipment and home modification recommendations, and implement practical intervention plans that align with each person’s goals and daily needs. Managing your own caseload, you will ensure documentation is accurate, outcomes are measurable, and services are delivered within agreed timeframes while working closely with clients, families, carers, and other stakeholders to achieve meaningful functional outcomes.
You will collaborate within a multidisciplinary team while also building strong relationships with referrers, community services, and support networks to ensure coordinated care. Alongside delivering high-quality therapy services, you will contribute to service growth through stakeholder engagement, help maintain performance targets, and support continuous improvement initiatives. You will play an active role in strengthening team culture, identifying opportunities to enhance service delivery, and ensuring clients receive timely, person-centred support that enables them to live as independently as possible.
Requirements- Minimum Bachelor degree in Occupational Therapy or relevant discipline
- AHPRA registration
- Police Check
- NDIS Screening Check
- Full driving license and access to reliable vehicle
