The Role:

Our client is seeking an experienced Manager – Coordination Services to oversee national programs across NDIS Support Coordination and Clinical Care Coordination. This is a leadership position with a strong focus on quality, collaboration, and impact. The successful candidate will manage a team of 15–20 staff, including Social Workers, Nurses, and Support Coordinators, while also holding a small caseload to remain connected to frontline service delivery.

Key responsibilities include:

  • Leading and coaching a multidisciplinary team to deliver high-quality, person-centred coordination services
  • Overseeing programs such as NDIS Support Coordination (Levels 2 & 3), hospital discharge support, GP Social Work, and complex mental health coordination
  • Ensuring compliance, quality, and performance across programs, including reporting and service outcomes
  • Supporting workforce planning, staff allocations, and recruitment oversight
  • Driving service innovation and contributing to research, data, and system reform

Requirements:

  • Qualifications: Degree in Social Work (preferred) with AASW membership, or Nursing with relevant experience. Accredited Supervisor status desirable (or willingness to work towards it).
  • 5+ years’ experience working with clients with severe or complex mental health needs (flexibility for strong candidates with slightly less).
  • Demonstrated leadership/management experience in health, disability, or community programs.
  • Strong understanding of NDIS, clinical care coordination, and community-based health systems
  • Clinical governance, compliance, and reporting expertise
  • Excellent people leadership, coaching, and stakeholder engagement skills
  • Ability to balance operational oversight with direct client practice
