The role:

As a Case Manager, you'll play a crucial role in building strong relationships with clients, stakeholders, and team members, while supporting clients with their individual needs and goals.

You'll have the chance to showcase your exceptional case management skills and develop consumer-directed care plans, ensuring alignment with company values and policies. With your exceptional communication and negotiation skills, advanced planning, organisational, and time management skills, you'll be able to respond to client feedback and manage client issues and outcomes with ease.

Your key responsibilities will include:

  1. Providing accurate and relevant information to both Home Care Package clients, private clients, and their families regarding their specific care needs.

  2. Ensuring compliance with legislative requirements while onboarding new clients, and developing personalized care plans and agreements for them.

  3. Conducting regular assessments and reviews with clients, utilizing best practice quality models for assessment, care plan development, implementation, and evaluation.

  4. Identifying clients who are vulnerable or at risk, and implementing appropriate legislative measures to ensure their safety and well-being.

  5. Understanding and following the necessary procedures to submit Serious Incident Response reports, if the need arises.

  6. Collaborating with rostering staff, support workers, external providers, and contractors as necessary to ensure effective client care.

  7. Coordinating the legislative documentation and visits of external contractors to clients' homes.

  8. Possessing a comprehensive understanding of key regulations such as The Aged Care Act, User Rights Principles, and The Home Care Operational Manual - Inclusions and Exclusions List.

Essential Criteria:

  • Minimum 2 years experience in Case Management with HCP and CHSP clients.
  • Demonstrated knowledge of the Aged Care Quality Standards for home care, The Aged care Act, The User Rights Principles and the Home Care Operational Manual 2023.
  • Strong communication skills, customer service skills, and demonstrated problem solving skills.
  • Current Drivers' License.
  • Access to fully insured car.
  • Knowledge of the Lookout Client Management System (beneficial)
  • Knowledge using MS office suite and My Aged Care services and assessments portal.
  • A deep understanding of the issues experienced by aged care clients and care staff in their home and the differing social, spiritual and cultural needs of each client.
