Make a difference as a Community Nurse in Sydney's Central and Eastern Suburbs

- $45.35 - $63.76 per hour including casual loading
- Regular rostered work available
- Shifts are 8:30am-4pm Monday-Friday
- Community role providing care to disadvantaged clients across Sydney's Inner and Eastern suburbs working for a mission driven not-for-profit
- Vehicle & phone provided for work
- Potential to transition into a permanent role with an organisation who may be able to offer sponsorship if required
- Working as part of a small team, provide high quality care to vulnerable people who are at risk of homelessness and experiencing chronic and/or complex health conditions
- Assess, implement and evaluate care plans as part of a multi-disciplinary team
- Take a leadership role in the coordination of nursing and health care and advocate for optimal outcomes, including where appropriate, making referrals to service providers and support services.
Essential Requirements
- Full rights to Work in Australia
- Registered Nurse with full AHPRA
- Current NSW driver's license (Class C)
- Experience in case planning and case coordination
- Mental health, addiction medicine or homeless health experience (highly desirable)
- Excellent communication and interpersonal skills in which to communicate with a diverse client group
- Demonstrated experience working within a strengths based framework to achieve agreed goals and outcomes.
- Strong administration and report writing skills
