- Fixed hours Monday to Friday, 8:00 am – 4:00 pm

As the Community Care Manager, you will be responsible for overseeing the delivery of high-quality, person-centred care services for clients living independently in the community. Your role includes conducting comprehensive client assessments, developing and reviewing personalised care plans, and ensuring services align with client needs and preferences. You will play a critical part in fostering strong relationships with clients, families, care staff, and external agencies to ensure seamless, culturally safe care delivery.
In addition to client-focused responsibilities, you will manage day-to-day operations, including budget oversight, staff supervision, and compliance with relevant aged care standards and legislation. Your leadership will cultivate a culture of continuous improvement and safety, enabling older adults to enjoy greater independence and wellbeing.
Requirements:- Tertiary qualification in Business Administration, Commerce, Nursing or related field (essential)
- Proven experience in operational or community care management, preferably within aged care or health services- Desirable
- Strong knowledge of aged care standards, funding models such as SAH, CHSP, or NDIS, and compliance frameworks
- Excellent leadership, communication, and budget management skills
- Valid Queensland driver’s licence (‘C’ Class or ‘LR’), First Aid and CPR certificates
- Satisfactory National Police Check and/or NDIS Worker Screening
- Current influenza and COVID-19 vaccinations (as per public health directives)
