We are seeking an experienced Grade 2 Social Worker to provide high-level clinical care, leadership, and supervision across a public health service. This role involves working collaboratively within multidisciplinary teams to deliver evidence-based assessment, intervention, advocacy, and discharge planning for patients and families.
You will play a key leadership role within the Social Work team, supporting junior clinicians and students, contributing to service improvement initiatives, and ensuring care aligns with best practice standards and organisational priorities.
Key Responsibilities

Deliver high-level clinical assessment, intervention, and discharge planning aligned with evidence-based practice

Participate in and lead multidisciplinary care planning , family meetings, and client/carer education

Manage a complex caseload across inpatient and/or community settings

Provide clinical supervision and professional support to Grade 1 Social Workers, Allied Health Assistants, and students

Contribute to quality improvement, research, and service development activities

Maintain accurate and timely clinical documentation and reporting

Build effective relationships with community agencies and external service providers

Participate in weekend and public holiday rosters as required
About You
You are a confident and experienced Social Worker with strong clinical reasoning skills and a commitment to person-centred, values-based care. You enjoy working collaboratively and are motivated to contribute to service excellence and continuous improvement.
Essential Criteria

Degree in Social Work eligible for full AASW membership

Demonstrated Grade 2 level clinical experience in assessment, intervention, and discharge planning

Ability to independently manage a complex caseload

Experience providing clinical supervision to junior staff and/or students

Strong written, verbal, and interpersonal communication skills

Excellent organisation, time management, and prioritisation skills

Commitment to ongoing professional development and reflective practice
Desirable

Experience in acute, sub-acute, or community health settings

Current Class C Driver’s Licence
