About the role:

As a Clinical Care Partner, you will be the key point of contact under the Support at Home Program. Your role involves developing and managing individualised care plans tailored to the unique needs, goals, and cultural backgrounds of older community members. Collaborating closely with participants, their families, and interdisciplinary teams, you’ll coordinate services focused on wellness, reablement, end-of-life care, and assistive technology. You’ll also ensure compliance with care standards, manage budgets, monitor risks, and provide education to support autonomy and improved quality of life.

You will work alongside clinically qualified professionals to escalate complex care needs when required, ensuring a high standard of care and seamless service delivery. This role demands strong communication skills and dedication to culturally safe and person-centred care.

Requirements:
  • Registered Nurse (RN) or Enrolled Nurse (EN) with current AHPRA registration
  • Diploma or equivalent qualification in Community Services, Aged Care, or Case Management
  • Proven experience in care coordination or case management within aged care or health services
  • Current Police Check or NDIS
  • Valid driver’s licence 
  • Strong knowledge of the Support at Home Program and aged care quality standards
  • Commitment to culturally responsive care and ability to work effectively with diverse communities
