As a Clinical Care Coordinator, you will assess residents' individual needs and develop tailored care plans that ensure the highest standard of clinical care. You will lead and support nursing and care staff to deliver safe, compassionate care while maintaining compliance with clinical standards and regulatory requirements.
Your responsibilities will also include direct nursing care such as medication administration, wound management, and clinical monitoring. Effective communication with residents, families, and healthcare professionals is key to this role, as is your participation in quality improvement initiatives and infection control measures.
Requirements:
- Bachelor of Nursing with current AHPRA registration
- Experience within a residential aged care nursing environment
- Strong clinical assessment, documentation, and leadership skills
- Effective communication and interpersonal skills
- Proficient computer skills, including Microsoft Office and clinical software
- Experience with aged care accreditation and quality improvement programs
- Current driver’s license
