The role:
As Clinical Care Coordinator, you will oversee all aspects of clinical care, including developing tailored care plans, leading a dedicated care team, and ensuring compliance with all regulatory standards. You will deliver hands-on nursing care such as medication management and wound care, while communicating effectively with residents, families, and healthcare professionals. Your leadership will ensure high-quality, safe care that aligns with best practices and enhances residents’ wellbeing.
Requirements:
- Bachelor of Nursing and current AHPRA registration
- Proven aged care nursing experience
- Strong clinical assessment and documentation skills
- Excellent communication and leadership abilities
- Proficiency with clinical software and Microsoft Office
- Valid Driver’s License (desirable)
- Ability to work collaboratively within a multidisciplinary team
