The role:
As a Clinical Care Coordinator based in the Hunter Region, you will play a pivotal role in delivering exceptional resident-focused care. You will lead clinical assessments, develop and oversee care plans, and ensure compliance with contemporary aged care standards. Working closely with nursing staff, allied health professionals, and families, you will coordinate healthcare delivery that promotes dignity, independence, and a safe environment for residents.

Your responsibilities will also include clinical supervision, mentoring junior staff, and supporting continuous quality improvement initiatives. You will be at the forefront of clinical decision-making, managing complex care needs, and ensuring communication flows smoothly among all stakeholders. This is an excellent opportunity for a skilled registered nurse looking to elevate their leadership and clinical coordination experience in aged care.

Requirements:
  • Current Registered Nurse Division 1 (NSW) with a minimum of 3–5 years clinical experience, preferably in aged care
  • Strong skills in clinical assessment, care planning, and medication management
  • Proven ability to supervise and mentor clinical staff
  • Excellent communication and interpersonal skills
  • Commitment to the Aged Care Code of Conduct and contemporary best practices
  • Willingness to undergo a Federal Police Check and comply with government vaccination requirements
Nice to have:
  • Postgraduate qualifications in aged care, gerontology, or leadership
  • Experience with AN-ACC funding model and aged care compliance frameworks
  • Prior experience in a senior clinical or coordination role
