The Role:

The Client Care Coordinator acts as a key operational link between Support Workers, Team Leaders, and the Client Care Manager.  You will provide hands-on oversight across multiple supported accommodation homes, ensuring consistent, high-quality service delivery. The role involves proactively identifying risks, issues, and opportunities for improvement. You will support and guide Support Workers to ensure person-centred practices are consistently delivered.  A key focus is maintaining clear communication between frontline staff, families, guardians, and the support office. You will ensure compliance with NDIS standards, internal policies, and regulatory requirements. The role also supports day-to-day operational coordination, including rostering input.  Overall, you will drive accountability, consistency, and a positive culture across all homes.

Essential Criteria:
  • Certificate IV in Disability, or 4 + years experience 
  • Demonstrated experience in a leadership or coordination role within disability or community services
  • Strong understanding of NDIS Practice Standards and the NDIS Code of Conduct
  • Experience supporting clients with complex needs and/or behaviours of concern
  • Excellent communication and interpersonal skills with the ability to engage diverse stakeholders
  • Proven ability to support, mentor, and motivate frontline teams
  • Strong organisational and operational coordination skills (rostering/workforce exposure desirable)
  • Current First Aid & CPR Certificate
  • Valid NSW Driver Licence
  • Current NDIS Worker Screening Check (or willingness to obtain)
  • Full Australian working rights
