The role:
As an Occupational Therapist working across Aged Care and the Support at Home Program (HCP/CHSP), you’ll play a key role in helping older adults maintain their independence, safety, and wellbeing at home and in the community. You’ll provide person-centred assessments and interventions focused on enhancing mobility, function, and engagement in meaningful daily activities. Using a holistic and strengths-based approach, you’ll work closely with clients, their families, and other allied health and care professionals to develop practical, goal-oriented plans that promote quality of life and confidence in day-to-day living.

In this role, you’ll manage a varied caseload across home and community settings, ensuring services meet both clinical best practice and the requirements of the Aged Care Quality Standards and Support at Home Program. You’ll maintain detailed documentation, monitor outcomes, and contribute to ongoing service improvement initiatives. You’ll also participate in regular supervision, professional development, and multidisciplinary collaboration to ensure clients receive coordinated, evidence-based care. Above all, you’ll bring compassion, professionalism, and creativity to your work—helping older Australians stay active, connected, and independent for as long as possible.

Requirements
  • Minimum Bachelor degree in Occupational Therapy or relevant discipline
  • General AHPRA registration
  • Valid NDIS Check
  • Police Check
  • Full driving license and access to reliable vehicle
