The Role
As the Youth Transition Worker, you’ll provide specialist outreach casework and care coordination to young people with complex needs — including AOD (alcohol and other drug) issues, mental health concerns, homelessness, and histories of trauma.
Working within a throughcare model, you’ll engage with clients up to three months pre-release and continue wrap-around support for 6–12 months post-release. The work will focus on building trust, strengthening cultural and family connections, and empowering young people to make positive life changes.
Key responsibilities:
- Provide intensive, holistic casework support to young people exiting custody
- Deliver culturally appropriate AOD and reintegration support services
- Undertake pre-release assessments and develop tailored case plans
- Support clients to access housing, education, employment, health, and counselling services
- Work collaboratively with families, carers, and community organisations to strengthen support networks
- Advocate for clients and help reduce barriers to community reintegration
- Apply trauma-informed, strength-based, and person-centred approaches in all interactions
- This is an Aboriginal Identified position. Applicants must identify as Aboriginal and/or Torres Strait Islander
- Demonstrated experience providing casework or youth support, ideally with clients exiting justice settings or facing complex needs
- Understanding of the issues affecting Aboriginal and Torres Strait Islander young people, particularly in the justice system
- Knowledge of AOD harm minimisation and relapse prevention strategies
- Experience in working collaboratively with families, communities, and multidisciplinary teams
- Strong communication, advocacy, and organisational skills
- Current NSW Driver’s Licence and Working with Children Check (or willingness to obtain
