The Role

We are seeking a dedicated and experienced Registered Nurse to join our team. In this role, you will be responsible for conducting nursing assessments in line with Department of Social Services (DSS) and Department of Human Services (DHS) requirements. These assessments will primarily focus on high-need service users, ensuring they receive specialist care plans to help them remain safely in their homes for longer. Additionally, you will provide nursing consultation and professional advice to community care staff and contribute to reviewing and improving clinical care protocols.

Requirements

• Relevant nursing qualification and registration
• Experience in conducting clinical assessments and care planning
• Strong understanding of DSS and DHS requirements
• Ability to collaborate with multidisciplinary teams
• Excellent communication and problem-solving skills
