Community Aged Care Registered Nurse - Perth, WA

• $65-80k  - negotiable for the right candidate

• Multi-disciplinary team with great Clinical support.  Work alongside Support Workers, Nurses, OT, Physio, GP, NP 

The role

• Provide culturally appropriate care to Aboriginal and Torres Strait Islander Elders returning home from hospital as part of a 12 week transitional care program.
• Work alongside Support Workers to deliver in home care including:
  • Pain management and assessment
  • Care for and manage various tubes (e.g., enteral feeding, nasogastric, tracheostomy).
  • Catheter and stoma care.
  • Manage complex wound cases as necessary.
  • Administer enemas or suppositories when needed.
  • Perform airway suctioning and tracheostomy care.
  • Supervise and administer oxygen therapy as required.
  • Manage medication appropriately.
  • Provide nursing services tailored to recipients' needs, including dementia support.
  • Take preventive measures to reduce falls among recipients.
  • Facilitate access to specialist nursing services on call, if necessary.
  • Facilitate the delivery of low-intensity therapies such as physiotherapy, occupational therapy, podiatry, speech pathology, counseling, and social work.
  • Aim to maintain and enhance recipients' physical and cognitive capabilities, as well as their independence in daily activities.

Criteria

• AHPRA registration as a Registered Nurse
• Australian drivers licence 
• Demonstrated ability to work collaboratively in a multidisciplinary team.
• Commitment to evidence-based practice and continuous quality improvement.
• Desirable: Experience in Aged Care / Community settings, and in delivering culturally appropriate care to First Nations people (training on this will be provided).
