The Job:
- We are seeking a compassionate Care Manager for an Aged Care Facility based in Geraldton to oversee the implementation and delivery of support & maintenance for residents.
- The role involves comprehensive assessment, professional case management, budget management within a Consumer Directed Care (CDC) framework, and coordination of services to meet clients' diverse needs.
- If you have a strong background in aged care, excellent communication skills, and a passion for ensuring high-quality, client-centered care, this role is for you!
Requirements:
- AHPRA Registered Nurse
- Relevant experience in aged care, community services, or complex case management
- Knowledge of the Home Care Packages Programme and CDC framework
- Ability to conduct comprehensive client assessments and develop tailored care plans.
- Proficiency in budget management and documentation
- Strong interpersonal skills with the ability to liaise with multiple stakeholders.
- Commitment to continuous professional development and adherence to organizational values and ethics
- Understanding of multicultural/diversity issues in aged care
