The Role
As a Case Manager, you will play a vital role in providing individualised case planning, coordination, and support to clients accessing social services. You’ll work closely with individuals and families with complex needs, collaborating with other service providers to ensure holistic, person-centred care.
Key responsibilities include:
- Conducting assessments and developing tailored case plans and exit strategies
- Facilitating access to a wide range of social support services
- Collaborating with internal teams and external stakeholders for integrated service delivery
- Applying Trauma-Informed Care and cultural safety principles
- Ensuring accurate client data entry and reporting
- Actively participating in community initiatives and inter-agency partnerships
Requirements
Essential:
- Tertiary qualification in social sciences or related field
- Proven experience in case management with individuals experiencing complex needs
- Demonstrated understanding of working respectfully with Aboriginal people and diverse communities
- Knowledge of Human Service Agencies and relevant legislative requirements
- Current NSW Driver’s Licence, Working with Children Check, and National Police Check
- Strong communication, negotiation, and problem-solving skills
- Proficiency in Microsoft Office and client data management systems
Desirable:
- Knowledge of regional service networks and partnerships
- Experience in measuring and reporting on key performance indicators
