In this position, you will also work with client availing of Home Care Packages (HCP). You will plan, implement, and evaluate individual therapy programs tailored to the unique needs of each client, ensuring documentation aligns with professional standards throughout the process. Collaborating closely with patients, families, support staff, and allied health services, you'll play a pivotal role in fostering a comprehensive approach to care.

Your responsibilities will extend to conducting both standardized and non-standardized assessments, generating functional reports to guide therapeutic interventions effectively. In addition, you will assess home and school environments, recommending aids, equipment, and modifications within funding guidelines to enhance clients' independence and quality of life. Communication and education will be key aspects of your role, as you provide pertinent information to clients, families, and support networks, facilitating a deeper understanding of therapeutic processes and outcomes.

Above all, your mission will be to make a tangible difference in the lives of our client's vulnerable community members on a daily basis. Guided by principles of Trust, Compassion, and Care, you'll help individuals overcome limitations stemming from injury, illness, developmental delays, or aging-related concerns. By empowering clients to transition from dependence to independence, you'll champion their personal productivity, well-being, and overall quality of life. While this overview outlines your primary responsibilities, you'll also be expected to adapt to evolving demands, embracing new tasks and opportunities that arise in line with the growth and development of our client's organization.
