We are partnering with a NFP organisation who's aim is to support the wellness of First Nation Elders. They are seeking a dedicated and experienced Occupational Therapist to join their dynamic team in their newly opened clinic in Kiwana. This is a rewarding opportunity to contribute to the health and wellness of Aboriginal and Torres Strait Islander communities across the state.

The Role:
As a Occupational Therapist within their Transitional Care Program (TCP), you will play a vital role in evaluating patients' physical conditions, developing tailored treatment plans, and delivering therapeutic interventions to enhance their physical abilities. Collaborating with a multidisciplinary team, you will provide holistic care, educate patients and their families on self-care techniques, and promote overall health and wellness within the community.

  • Conduct comprehensive assessments of patients' functional abilities, including activities of daily living, mobility, cognition and psychosocial factors..
  • Utilise standardised assessment tools and clinical observations to evaluate patients' strengths, limitations, and rehabilitation potential.
  • Develop individualised treatment plans based on assessment findings, patient goals, and discharge plans.
  • Implement evidence-based interventions to address patients' occupational therapy needs, such as therapeutic exercises, adaptive equipment training, environmental modifications, and cognitive rehabilitation.
  • Collaborate with interdisciplinary team members, including physiotherapists, nurses, and social workers, to coordinate care and optimise patient outcomes.

Criteria:

  • Bachelor's degree in Occupational Therapy.
  • Full AHPRA Registration.
  • WA Driver’s license.
  • Police Clearance (no more than 3 months old).
  • Vulnerable Peoples check.
  • First Aid certification or willingness to obtain.
  • COVID-19 Vaccination Certificate.
  • Experience working with First Nations people is desirable.
  • Ability to deliver culturally appropriate services for First Nations Elders.
