The Role:
This role focuses on leveraging strong community development and program management skills to craft effective strategies for meeting predefined targets and goals. Leadership and management capabilities are crucial, alongside an advanced understanding of medical diagnoses and associated social health issues within the community. While familiarity with Australian primary health care systems and national mental health standards is advantageous, it's not a strict requirement. Knowledge of the long-term impacts of physical and mental health issues, as well as awareness of community and health support services in our clients’ catchment area, is essential. Experience in cross-cultural settings and proficiency in a second language are assets. The incumbent should excel in establishing and nurturing clinical and community relationships, including those with general practices and health services in the local area.

Criteria:
- Bachelor of Social Work
- Knowledge of the Australian primary health care systems and national mental health standards
- Strong Community Development and program management skills and the ability to develop strategies to meet specified targets and outcomes.
- Leadership and management skills, with a demonstrated advanced knowledge base specific to medical diagnoses and related social health issues presenting in the community
- Drivers Licence (You will ideally have access to your own vehicle as well)
