Community Nurse

• $38.86-  $54.56 per hour
• Regular rostered work available - would suit someone looking for 2-4 days per week
• Shifts are 8:30am-4pm Monday-Friday 
• Community role providing care to disadvantaged clients across Sydney's Inner and Eastern suburbs working for a mission driven not-for-profit
• Vehicle & phone provided for work
• Potential to transition into a permanent role with an organisation who may be able to offer sponsorship if required

The role 

• Working as part of a small team, provide high quality care to vulnerable people who are at risk of homelessness and experiencing chronic and/or complex health conditions 
• Assess, implement and evaluate care plans as part of a multi-disciplinary team 
• Take a leadership role in the coordination of nursing and health care and advocate for optimal outcomes, including where appropriate, making referrals to service providers and support services.

• Registered Nurse with full AHPRA
• Current NSW driver's license (Class C)
• Experience in case planning and case coordination
• Mental health, addiction medicine or homeless health experience (highly desirable)
• Excellent communication and interpersonal skills in which to communicate with a diverse client group
• Demonstrated experience working within a strengths based framework to achieve agreed goals and outcomes.
• Strong administration and report writing skills
