As a Mental Health Nurse within the Primary Integrated Care Supports (PICS) Program, you’ll deliver evidence-based, person-centred care to individuals living with severe and enduring mental illness. The role combines clinical expertise with a recovery-oriented, trauma-informed approach. You'll be part of a multidisciplinary team of nurses and peer workers supporting clients through comprehensive care coordination and community integration.
Key responsibilities include:
- Accept and review referrals from Intake, conducting due diligence
- Deliver quality, timely and culturally appropriate mental health care
- Establish therapeutic, recovery-focused relationships with clients
- Involve families and carers (with consent), providing support and referral
- Coordinate holistic physical and mental health assessments and care
- Support access to treatment and monitor health outcomes
- Work collaboratively with GPs, Psychiatrists, Allied Health and community services
- Maintain accurate clinical documentation in Salesforce and Redicase
- Engage with psychosocial supports according to individual client needs
- Participate in supervision, nurse practice meetings, and Communities of Practice
- Adhere to ACMHN Codes of Ethics and Practice, One Door and PHN policies
- Use interpreting services when working with CALD individuals
- Collaborate with Peer Workers and Peer Work Coordinator
Essential
- Credentialed with the Australian College of Mental Health Nurses (or working towards credentialing within 12 months)
- Minimum 5 years’ experience as a mental health nurse in a community setting
- Willingness to complete Cultural Competency and Trauma-Informed Care training
- Experience working with GPs and community-based psychiatrists
- Ability to self-manage clinical practice and work both autonomously and in a team
- Comfortable working alongside and supporting peer workers
- Flexible and creative in supporting individuals with complex needs
- Proficient in internet-based Client Management Systems (e.g., CiMS)
- Valid National Police Check, Working with Children Check, NDIS Worker Screening, and current COVID-19 vaccinations (or willing to obtain)
- Experience working with clients from diverse cultural backgrounds
- Addiction Experience
