The Role

Our client is seeking an experienced and motivated Registered Nurse & Clinical Assessor to join their expanding team. This rewarding role focuses on clinical leadership and assessment across Home Care Packages (HCP), NDIS, and Supported Independent Living (SIL) programs.
The successful candidate will play a key role in ensuring high-quality care outcomes, leading clinical assessments, and contributing to governance and compliance processes. The position involves working closely with case managers, allied health professionals, and families to deliver tailored and effective care plans.

Key Responsibilities:

  • Conduct comprehensive assessments for HCP, NDIS, and SIL clients
  • Develop and review personalised care plans in collaboration with multidisciplinary teams
  • Support ongoing clinical monitoring, reassessments, and incident management
  • Participate in quality assurance, audits, and continuous improvement initiatives
  • Provide clinical advice and onboarding input for new clients
  • Deliver clinical support and guidance to support workers as required
  • Maintain communication with external providers including GPs and allied health

Key Focus Areas:

  • 80% Aged Care (HCP & CHSP)
  • 20% Disability (NDIS & SIL)


Requirements

Essential:

  • Registered Nurse (Division 1) with current AHPRA registration
  • 4–5 years of experience in aged care and/or disability services
  • Proven skills in clinical assessments, care planning, and documentation
  • Strong understanding of clinical governance and sector compliance
  • Excellent interpersonal and communication skills
  • Computer literacy and experience with clinical systems
  • Police Check, WWCC, and NDIS Worker Screening
  • Current driver’s licence and access to a reliable vehicle

Desirable:

  • Experience in clinical governance or quality improvement
  • Familiarity with My Aged Care, PRODA, NDIS portal, or care management software
