About the Role:
Our client has an exciting opportunity for a skilled and motivated leader to fill the role of Case Manager. The Case Manager is a front-line management role, responsible for the day-to-day operations of one Non-Family Based Care Program.

Key Responsibilities of this role include:
  • Manage and oversee the day to day running of the Non-Family Based Care Program
  • Provide leadership and management of all staff members within the Program
  • Ensure that the Program consistently provides a hygienic, safe and therapeutic environment for young people to a high standard
  • Provide trauma informed therapeutic care to young people that meets the Standards of Care and the organisations Policies and Procedures
  • Manage positive and respectful relationships with all internal and external stakeholders
  • Ensure the Program adheres to all reporting and documentation requirements
  • Write Safety Plans and contribute to Positive Behaviour Support Plans with the support of the Practice Advisor
  • Facilitate/attend relevant meetings including household and stakeholder meetings
  • Attend critical appointments with young people as a support person and advocate for them
  • Ensure rosters meet industrial compliance and are completed 2 weeks in advance
  • Plan and conduct developmentally appropriate individual and group activities consistent with young people's needs and case plans goals
  • Coordinate efficient and comprehensive on-boarding of new staff members.

Criteria:
  • Experience working within a leadership / senior position across the welfare remit
  • Diploma level qualification (Minimum)
  • Excellent interpersonal and communication skills, with the ability to build and maintain effective relationships with all stakeholders
  • Demonstrated leadership abilities, including the capacity to motivate and empower a team to achieve positive outcomes
  • Exceptional time management and prioritisation skills
