Role:

As a Care Services Coordinator, you will collaborate with individuals who have faced significant injuries, supporting them on their path to recovery and return to work or home. Utilising your expertise in physiotherapy, occupational therapy, or nursing, you will assess their needs, develop personalised care plans, and coordinate and case manage treatment and care.

Your responsibilities will include overseeing their rehabilitation process, ensuring regulatory compliance, and building positive relationships with clients and healthcare providers.

Requirements:

• Relevant degree in Physiotherapy, Occupational Therapy, or Nursing
• Current AHPRA Registration 
• Experience in FIM and CANS 
• Experience in case management 
• Relevant check such as NDIS workers screening, police check etc.
