A well established Aged Care provider is looking for a care manager to join their team at their centre 15 minutes from Perth CBD.

The Role:
  • Support Facility Managers in coordinating healthcare services for residents with a focus on patient centred care
  • Medium Sized Facility
  • Supervise Registered Nurses, Enrolled Nurses, and Care Workers
  • Coordinate Continuous Improvement, Occupational Health and Safety, and Training/Orientation programs
  • Monitor and identify AN-ACC reviews for optimised consumer care funding
Requirements:
  • Current unrestricted AHPRA registration
  • Bachelor of Nursing or equivalent qualification
  • Prior clinical care coordinator or care manager experience
  • Strong leadership and communication skills
  • Knowledge of AN-ACC and Aged Care legislation
  • Clear National Police Check
  • BLS & NDIS 
  • Valid Australian Working Rights
